WHAT AGENCIES ARE PROVIDING SUCH FINANCIAL ASSISTANCE

- City/Municipal Social Welfare and Development Offices (C/MSWD) of every Local Government Unit – All Cities/Municipalities are providing support services particularly financial assistance through the Assistance to Individual in Crisis Situation Program (AICS) Program of the C/MSWD).

- Social Welfare Action Desk (SWAD)- The Department of Social Welfare and Development is providing support services to medically ill patients, confined and those who are in need of burial assistance through their Crisis Intervention Unit particularly the SWAD Offices. For clients maintaining lifetime medications, they can seek assistance every after three months.
(Ang DSWD sa pamamagitan ng Crisis Intervention Unit particular na ang SWAD Offices ay nagbibigay ng tulong pinansiyal sa mga nabanggit na kliyente)
- Party lists and Office of the Congressman (Representative)- You may also seek assistance through this Offices however their fund is downloaded to the nearest SWAD Offices. Since the Disbursing Officer is the SWAD Office, you can either choose one between SWAD Office, Partylist and Congresswoman. For clients maintaining lifetime medications, they can seek assistance every after three months.

Subalit ang kanilang pondo ay nakadownload sa tanggapan ng DSWD sa pamamagitan ng SWAD. Ibig sabihin ay maari lamang humingi ng tulong sa isa sa mga nasabing ahensiya. - Philippine Charity Sweepstakes Office- Their fund is limited only for hospitalization. Assistance provided are in the form of guarantee letters.
( Ang tanggapan ng PCSO ay limitado lamang sa pagbibigay ng tulong pinansiyal sa mga may balanse pa sa kanilang hospitalization. Dahil ang tulong na ibinibigay ng ahensiyang ito ay sa pamagitan lamang ng Guarantee Letter na siyang ibabayad sa hospital)

- Barangay Certificate of Indigency
- Original Copy of the Medical Certificate/Clinical Abstract
- Temporary or Final Bill/Promissory Note duly Notarized for Hospitalization (Original)
- Original Copy of the Prescription for Medicines
- Treatment Quotation for Hemodialysis and Chemotherapy
- Xerox Copy of Valid I.D of the Patient and the Person Seeking assistance
- Endorsement Letter from Partylist or from the Office of Congressman/Representative
- Social Case Study Report or General Intake Sheet from the C/MSWD
- Xerox Copy of Cedula (Additional Requirement for AICS in Santiago City)

- Barangay Certificate of Indigency
